Dish adapter only worked once?

Decided to head down to France this bank holiday.
I was able to source a dish adapter and was able to successfully top the car up. I did end up dropping the adapter from a low height - no visible damage.

Upon the second attempt to top up, the dish pump would latch one but gas would immediately start leaking, due to the valve being open. Whether this was due to the drop or a faulty pump, I could not know.

Assuming it was indeed a faulty pump, I moved onto another station and the same thing happened. Now, as I am currently typing this from the ferry, I am yet to get home to test our bayonet pumps to see what is at fault. Perhaps I damaged the dish adapter, or the bayonet connect itself.

What could possibly be the reason of failure?

Upon reading that thread, one possibly could have been due to the weight distribution of the pump upon the dish adapter.

Some of the French pumps are well worn and you need to keep hold of the hose to take the weight off the nozzle. Although I haven't had that problem for a while now, I think they are maintaining their pumps a bit better. Either that or you have bent the adapter so they don't seal properly. Where were you filling up? I was over last week and filled up at Carrefour in Calais (0.99 Euro a litre) twice (once as soon as I got off the ferry and again on my way back just before getting on the ferry) and again at a Total station near to Osny (1.03 Euro a litre), no problems at either.
